I always feel welcomed when I come to visit my father. Knowing that he’s comfortable and well taken care of gives me a peace of mind. Tan Evans has been so helpful with addressing any issues or questions we have had regarding my father.

My loved one with frontotemporal dementia stayed here for what was supposed to be a brief weekend respite stay in April 2023. Everything went wrong from the start. They were not prepared for her arrival, they had absolutely no activities planned for any of the days she was in residence, and when I visited during visiting hours there was only one nurse (a young man on his first day) on the floor. I refused to leave until they addressed the under-staffing issue until a second nurse came to the floor approximately 2.5 hours later (11 p.m.). Subsequently, they moved my loved one to a different room on a different floor, taking away and then losing all of her possessions including her purse, all without notifying me. What was supposed to be a respite stay for us turned out to be anything but. A name change will not rid this place of the poor reviews they deserve. I don't blame the staff who show up to do their best, but the management creates an environment where the staff clearly cannot provide even the minimum care their residents deserve. If you visit, stay for a while, see how it goes. Visit in the evening and push the call button. I waited 45 minutes with no one coming until I went to find them. Avoid at all costs.

I visited my elderly relative here. She seemed happy. The place seems clean & staff were nice. I didn't see her room so can't say more.

The few times I visited the place, while my aunt was recovering there, everyone was professional and on task. The staff seems to be well trained. The social worker Rasheed will keep you informed and he's always available.

My dad was just transferred to Symphony of Lincoln Park. I didn’t know what to expect, because I have no first hand knowledge of any nursing home/short term rehab facility. I spent several hours with my dad and several nurses, his case manager and other staff. From my first introduction to the facility by the reception desk, I had a good feeling about the place. I am so thankful that their staff truly cares about the health and well-being of my dear dad. As his primary caregiver, it is hard for me to leave him when visiting hours end, but I know that I leave him in kind and capable hands. Thank you, Symphony of Lincoln Park!

The two black girls and the philippine girl on the 2nd shift are awful. They have a very very bad attitudes .There rude and act like they can't stand the patients. I was always nice to them but they always blue me off.They shouldn't be nurses.They stink and it shows.A lot of the patients don't like the way they treat everybody and they should be fired

Terrible facility! My neighbor went to Symphony for rehab after her hospital stay. She is 94 and walked fine with no assistance. She was in the hospital for 3 weeks they thought it would be great to send her here for rehab to strengthen her legs instead they drugged her so she was unable to participate in her rehab sessions. They discharged her in a wheelchair unable to walk. So disappointing.

My Mother has been at this Nursing Home since December 2020. She was initially in Short-Term Care, they have recently put her in Long-Term care. The rooms and the level of care she is receiving in Long-Term care is very different from Short-Term care. The rooms are awful, in her room there were towels behind the toilet in the bathroom, the towels appear to have been down there for weeks because they were brown. I mentioned this to a staff member and instead of them fixing the toilet they put more towels down on the floor. I have since talked to the VP of Operations and he had maintenance come in and fix the toilet, however they did not fix the walls, floor and the baseboards around the toilet, they were damaged due the leaking toilet and moisture from lack of ventilation in the bathroom. The wall paper and baseboards in the room are literally coming off of the walls and they have them secured with thumb tacks. So it would not look so bad, I removed the thumb tacks and brought in a staple gun and secured the wall paper, I was not able to secure the baseboards. Upon arrival in the room the floor tiles around my Mother's bed were cracked and I was literally tripping when I would walk toward her bed. Again I talked to someone in Administration, I cannot remember who it was and maintenance did fix the floor tile, however they did such a poor job the glue they used was oozing out of the sizes of the tile and now the floor is sticky. As far as the level of care, my Mother developed bed sores, she kept complaining her back was hurting and I started rubbing her back, I found blisters and sores on her back. I was livid, I told the nurse in charge and she act if so this was not a problem and said they would address it the next day, I insisted they address it that day and told them I did not care that it was 8:00pm, I was not leaving until they put some type of cream and badges on her back. She finally saw I was not playing with her and she treated her wound. Also I complained to the VP of Operations and told him she is not able to move around on her own, they should have someone move her every few hours to prevent this from happening. This concerned me because this is a Nursing Home why did I have to inform them of their job. Also she and other patients are just siting there for hours with no one checking on them, most of the nurses and CNA's are sitting around on their cell phones all day. I am strongly considering moving her to another Nursing Home, but I am afraid there will be no difference in the level of care she is receiving. It is a shame that the State gives these places money and they treat our Elderly Family members this way. There should be some level of oversight for these facilities. Do not be fooled by the aesthetics of the lobby or the Starbucks at this Symphony location, the view is much different when you go into the rooms on the Long Term Care floors.
